AFFIDAVIT

I Mr. Rajesh Kumar solemnly declare and affirm the


following:
1. That I Rajesh Kumar (Passport No. ………) married
to Mrs Priyanka Mishra (Passport No. …..) under Hindu
Marriage Act and residing presently at Flat no 405
Dhwisha Apartment Plot no 49 , Sector no.17 , Roadpali,
Taluka Panvel Raigad 410218
2. That I got Married on 30th April 2015 at Azamgarh, Uttar
Pradesh with Mrs Priyanka Mishra. I got married as per
as per Hindu rituals and I have done all my vidhis as per
Hindu Marriage Act.
3. That as a proof of my marriage, I have a valid marriage
certificate. I have completed my pheras, or saptapadi, in
presence of fire. Photographs were taken throughout the
event, however some of the Pheras photos are
unavailable. However, I am providing some tentative
pictures of Sindurdani and Pheras that I have on
available. However, I have a declaration indicating that I
misplaced some of the Pheras photographs.
