PERSONAL AFFIDAVIT
I, JERICHOB NARRA, of legal age, Filipino, single, and a resident of
Block 2 Lot 13 Sto. Nino St., UPS-5, San Isidro, Paranaque City, after having
been duly sworn to in accordance with law, do hereby depose and say:
1. That I am the affiant herein with birth name JERICHOB NARRA,
that was born on July 26, 2001 at Manila City;
2. That the said birth name was also appearing in my Certificate of
Live Birth registered at the Local Civil Registry of Manila City;
3. That in my Form 137, my name was erroneously recorded as
JERICHOB ANICO NARRA, that the said discrepancy was made due
to wrong entry of my birth name;
4. That to remove confusion, my birth name, JERICHOB NARRA,
that was appearing in my Certificate of Live Birth and the name,
JERICHOB ANICO NARRA, that was appearing in my Form 137
is referring to one and the same person and JERICHOB NARRA
is my true and accurate birth name;
5. That I execute this affidavit to attest to the foregoing facts and
for whatever purposes it may serve.
