JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, KRISTINE JOY MATEO SANCHEZ, of legal age, Filipino Citizen, and resident of C-34 GK-
Aya Timothy St., Multinational Vilage, Moonwalk, Paranaque City, and RIMBOY ORQUITA, of legal age,
Filipino Citizen, and resident of Unit 2 GK-Aya Timothy St., Multinational Vilage, Moonwalk, Paranaque City,
after having been duly sworn to in accordance with law do hereby depose and state that:

1. That we personally know RITA ORAY DEL MONTE born on August 13, 1965 who was born on San
Policarpo, Eastern Samar, who is our long-time acquaintance in Parañaque City;

2. That we know for a fact that the above-mentioned name is her true and accurate name and is
appearing in Certificate of Marriage between her husband, ROGER C. DEL MONTE of registered
at the Office of the Civil Registrar – Municipality of Paranaque City and other Valid Identification
Cards.

3. That upon checking for the record of birth of RITA SASMA ORAY, her maiden name at the
National Statistic Office presently known Philippine Statics Office. The National Statistic presently
known Philippine Statics Office issued a Certification that certify that RITA SASMA ORAY has “NO
RECORD OF BIRTH”.

4. That this NO RECORD OF BIRTH at National Statistic presently known Philippine Statics Office
has caused discrepancies and confusion in transactions and processes requiring verification of
identity;

5. That we attest to the fact that the Subject’s correct and true name is RITA ORAY DEL MONTE,
based on the valid identification documents and Certificate of Marriage presented to us;

6. That this affidavit is being executed to attest to the truth of the foregoing facts and to support the
correction of the Subject’s name and to avoid further inconvenience and discrepancies;

7. That we are executing this affidavit freely and voluntarily to attest to the truth of the foregoing facts
for whatever legal purpose it may serve.
