Affidavit of Lost Bank Passbook

Atty. Jesus Maria James L. Bringas swore in an affidavit that he is the holder of a Philippine National Bank savings account pass book issued to the Integrated Bar of the Philippines, Agusan del Norte Chapter. He states that he kept the pass book since being elected Treasurer of the chapter. However, he discovered in 2008 that the pass book was no longer in its usual place and could not be located despite searching. He executed this affidavit to attest to these facts and request that a new pass book be issued to replace the lost one.

AFFIDAVIT OF LOSS

I, ATTY. JESUS MARIA JAMES L. BRINGAS, of legal age,


Filipino, married and resident of Libertad, Butuan City, Philippines,
after having duly sworn according to law, depose and say:

1. That I am the holder of Philippine National Bank Savings


Account No. 285-906600011 Pass Book of Integrated Bar of the
Philippines, Agusan del Norte Chapter.

2. Upon the issuance of the said Savings Account Pass Book I kept the
same since I was elected Treasurer of the chapter.

3. That sometime on 2008, I discovered that the Savings


Account Pass Book issued by the above-mentioned Bank was no
longer in its usual place and can no longer be located despite diligent
and extensive search;

4. That I am executing this affidavit to attest to the truth of


the foregoing facts above-stated in order that a new Saving Pass Book
be issued to replace the same and for no other purpose contrary to
law.

FURTHER AFFIANT SAYETH NAUGHT.
