FORMAL LETTER OF REQUEST

Dear Maam/ Sir:

I am, Edgardo S. Uguis, a resident of 128-A Maria Clara St., Tagbilaran City,
Bohol. Last January 24, 2018, I was served a copy of a termination letter signed by,
Allan Ricardo Real, the Punong Barangay of Poblacion 2, Tagbilaran City, regarding
my dismissal as a Barangay Treasurer.

While I already have raised my complaint over such termination before the
Sangguniang Panglungsod of Tagbilaran City, I am writing to you this formal letter to
request that, pending investigation of my complaint, I be granted a suitable
opportunity to liquidate the unliquidated vouchers I issued during the time I held
the position as Barangay Treasurer from November 2017 to January 31, 2018.

Considering that I was abruptly informed of my termination, which then


became effective few days after, I never had an ample time to do the liquidation. I am
greatly concerned about this as those vouchers were issued under my name and
capacity as then Barangay Treasurer.

I believe that it is but my right that I be granted this, most especially as this
will help me clear my name from whatever alleged anomalies that were accused of
me. Thank you for the time and I hope you consider my request.
