Sequoia 15.7.3 (M4 Pro), using the extraMkvTypesSupport build.
I haven't been able to identify any particular pattern, but the QLC process randomly hangs when viewing MKV files. It shows in Activity Monitor as "Not Responding", and QL previews then just show a spinner, both in the Finder pane and when QL is activated. It's at least alive enough to respond to Quit and not need Force Quit, though.
Obviously, while it seems to be triggering on MKVs (because that's what I have), once it hangs it's stuck for all QLCodec-handled types. In case it matters, a lot of these are VP9+AAC.
