Please add dsh-model-memory to Memory section #76

Hi! Could you please add dsh-model-memory to the "Memory" section?

A direct pull request was not possible from my fork (unrelated git history), so here is a ready-to-paste entry:

• Mutx163/dsh-model-memory — Reasoning-effort tier management for custom API models (inline Settings -> Models toggles, atomic settings.yaml writes) with cross-session per-channel memory that auto-restores the last model and effort level.
