This pull request fixes #50 and #51, and allows sokol_gp.h to be used with the latest version of sokol_gfx.h.
It also adds reference based functions equivalents for
sgp_setup(),sgp_make_pipeline()andsgp_set_uniform(), for easier integration with C++ code.
