When fetching refs with paths such as other.json#/some/path, use jsonpointer to resolve #/some/path and store just that part of the object in the refs map.

Before I attack any issue that has no PR attached, I tried to merge in all the PRs already in existence.
I've created a
Changes.mdto document the changelog for the new release. (I've picked v1.1.0 so that we'll able to seamlessly merge in with the version living under npm down the line).All the tickets related to this release are under the milestone 'NEXT RELEASE' (https://github.com/lbovet/docson/milestone/1?closed=1).
