I played a game today in which a cosmonaut was killed in a training accident, but wasn't removed from the flight crew until the following turn (when another cosmonaut happened to die in a train wreck). However, that second guy was removed from the Vostok program right away. (As I recall, this sort of thing also happened in the old CVS code.)
