Did you closely follow the installation instructions? Did you download the correct version? If the (honest) answer to those questions is 'yes' you can ask for help in the obs forums thread.
Generally the answer is no. I made a tool, which you can use to create your own overlays. If you run into issues with that (after watching the tutorial video for it) feel free to ask me though.
Unless it's very trivial I don't plan on adding anything after 5.0. You can still submit a request, although I can't guarantee anything.
Yes and no. The only dependency I use is compatible with macOS, but I won't spend any time myself on porting it. So as of right now there's no way of using this on macOS.

I don't know. I only have one gamepad to test with and anything beyond that is a mystery to me. If your game pad works in the config creation tool but doesn't work in obs you can try following these instructions to load your gamepad preset in the obs browser source, this defers gamepad input handling to Chromium (i.e. a web browser) which usually work (maybe).
