3 last-mile fails I see

1. Handoff owner missing — Step 5 report + state.json checkpoint update, but no named human for "AGENTS.md needs confirm" timeouts. Risk: delta scan waits forever on [y]/.
2. Acceptance not machine-checkable — "accurate, project-specific documentation" has no PASS/FAIL rubric (coverage %, required sections present). Risk: partial project-doc.md marked complete.
3. When-not-to-use / blast radius — Full scan can rewrite agent instructions; no hard stop for monorepos / multi-package roots. Risk: wrong stack inferred → bad AGENTS.md shipped.
