AFFIDAVIT OF CONSENT

I, AAA, of legal age, widow, Filipino and presently residing at Brgy. Liwan
Norte, Enrile, Cagayan, and after having sworn to in accordance with law, hereby
depose and state:

1. That I am the bondsman of accused BBB in Criminal Case No. 123456,


lodged at RTC 10, Cagayan de Oro;

2. That accused filed his Notice of Appeal, and I hereby give my consent, that
the said bail bond which I posted for his temporary liberty be used as his
bail bond pending Appeal to the Court of Appeal, so that he could be a
productive citizen in the community;

3. That accused children was still young and in the formative stage and it
would be hard for them to be forsaken by reasons of this case without a
fight;

4. That what the accused wanted, is to have a chance of a life time to appeal
his case and be with his family pending appeal of his case which I readily
give my consent;

5. That accused is ready, able, and willing to abide with such other order that
this Honorable Court, may imposed upon him when his motion to release
pending appeal be approved;

6. That I am executing this affidavit in order to attest to the truth and veracity
of the foregoing statement.
