AFFIDAVIT COMPLAINT

I, SANTIAGO S. BALISI, of legal age, married, Filipino and


presently residing at 57 A. Adduru St., Annafunan West, Tuguegarao
City, Cagayan and after having sworn to in accordance with law, hereby
depose and state:

1. That I was employed by Sara International Manpower Services,


Inc., with Office address at S22-25 2 ND FLORR Midland Plaza
Condominium, M. Adriatico, Ermita, Manila, and Maricar J.
Monson is the owner/manager;

2. That I was able to land a job thru the said Agency, and my
employer was Alhamrani Tradingand Import Company at Riyadh,
Saudi Arabia, wherein a contract was executed wherein I
supposed to received US$ 800.00 per month or 4,150.00 Riyals
per month including the housing and transportation allowances.
The contract was good for 2 years and with authomatic renewal of
contract;

3. That I was able to finished the 2 contracts to my employer, 0r


for 4 years I was able to complete the 2 contracts. However for the
3rd contract, I was able to rendered 10 months with pay and
additional 19 days which they allowed me to worked but they
failed to pay all corresponding monetary consideration due to me
and they terminated me for unknown reason;

4. The sad thing is that they maltreated me, with physical violence
upon my person, they failed to pay my salaries, separation pay,
housing allowance, transportation allowance, and salary for the
last 19 days which I rendered for work and breach of contract
because, I’m supposed to complete the 2 years contract;
5. That I am executing this affidavit in order to attest to the truth
and veracity of the foregoing statement;

FURTHER AFFIANT SAYETH NAUGHT.

IN WITNESS WHEREOF, I have here unto affix my hand this June 5,


2015, here at Tuguegarao City, Cagayan.
