AFFIDAVIT OF RELATIONSHIP

been duly sworn to in accordance with law, do hereby
depose and say:

Certificate hereto attached and marked as Annex “A”, to form
part hereof;

That I am executing this affidavit to attest to the truth


of the foregoing facts, and to prove my relationship with my
