I, DATU RASUL DENILA, of legal age, Filipino citizens, resident of _____


respectively, after having been duly sworn to in accordance with law to hereby depose
and say that:

1) I am the son of NOREEN DACULA ABDULA and EFREN DENILA;

2) I am the only son born from the marriage of NOREEN DACULA ABDULA
and EFREN DENILA;

3) I have a half sister named CAZZEL MAE DENILA who is the daughter of
EFREN DENILA;

4) CAZZEL MAE DENILA is not in anyway related by affinity or consanguinity


to NOREEN DACULA ABDULA;

5) I am executing this affidavit to attest to the truthfulness of the above-stated


facts and in support of my mother’s claim for PAG-IBIG arising from her
death.

Sultan Kudarat, Maguindanao, after having been duly sworn to in accordance with law to
hereby depose and say:

1) That I am the son of NOREEN DACULA ABDULA,

2) That I personally know here to use both the names of “BAI NOREEN” and
“NOREEN” in her official records interchangeably;

3) That Identification cards with name “BAI NOREEN” refer to her as one and
the same person;

4) That I have executed this affidavit to attest to the truthfulness of the foregoing
facts.
