A SANGGUNIANG BARANGAY RESOLUTION RECOMMENDING TO THE PUNONG


BARANGAY THE MODE OF PROCUREMENT TO BE ADOPTED AND UNDERTAKEN FOR
PROCUREMENT OF GOODS, SUPPLIES, MATERIALS, EQUIPMENT AND SERVICES

WHEREAS, a request for quotation was sent to (name of stores)

WHEREAS, the (name of store) submitted a quotation of Php.

WHEREAS, on (price quote) the BAC members together with the BAC Secretariat and BAC
Technical Working Group, convened for the opening of price quotation submitted for the
procurement.

WHEREAS, the procurement was conducted through small value procurement.

NOW THEREFORE, we the members of the BAC hereby RESOLVE to recommend to the
Punong Barangay to award to (name of store) to supply and delivery of _________________

RESOLVE FURTHER, to furnish copy of this resolution to all agencies concerned for their
information.

I hereby certify the correctness of the foregoing resolution which was duly attested
and approved by the Sangguniang Barangay during its regular session held April 01,
2019.
