AFFIDAVIT OF AUTHORIZATION

I, Esmeralda M. Olita, of legal age, Filipino, and a resident of San Joaquin,


Macrohon, Southern Leyte, Philippines, after having been duly sworn to in
accordance with law, depose and say:

1. That I am the absolute owner of a parcel of land, located at San Joaquin,


Macrohon, Southern Leyte, designated as Lot No. 1-A-Portion, with an
area of 250 square meters, with Tax Declaration No. 07-08-021-00088,
which I bought from Alexander T. Mesa;

2. That my son, Exequeil Jarabe, III, is applying for a Building Permit at the
Office of the MPDC of the Municipality of Macrohon, so he could build his
residential house on the aforementioned parcel of land;

3. That I hereby signify my written consent, freely, willingly and voluntarily to


allow my son, Exequeil Jarabe, III to build his residential house, and
consequently apply for an electric connection therein;

4. That I am executing this affidavit to attest to the veracity of the foregoing,


and to support his application for a Building Permit and Electrical Permit.
