AFFIDAVIT

I, ANGELITA Q. IDOLOG, of legal age, Filipino with resident at 147 Buayang Bato,
Mandaluyong City, Philippines, after having been duly sworn in accordance with the law do
hereby depose and state THAT:

1. That I am owner of a parcel of land located at the Municipality (now City) of San
Juan, Philippines and is presently covered by Transfer Certificate of Title No.
3803

2. That my father Rafael Quinicio Sr. passed away on January 24, 2000 and my
mother Estilita R. Quinicio passed away on June 3, 1993, whereas I have
hereunto attached the Death Certificate marked as Exhibit “A”,

3. I applied with the Manila Electric Company “MERALCO” for the reconnection of
the electric service in the Subject Property and as such was required by the latter
to execute a Deed of Undertaking to the jointly and severally liable with the
occupant of the Subject Property for unpaid regular monthly bills incurred by the
occupant but not to exceed two (2) months;

4. However, the Subject Property has not been extra judicially settled among the
lawful heirs and for this reason I was appointed to administer the Subject
Property pending the execution of the settlement;

5. I hereby declare that there is no one from among the other heirs who has any
objection to my application with MERALCO and that they do not have any claim
or interest on the Subject Property averse to mine;

6. I likewise hereby hold MERALCO, free and harmless of any liability or damage by
reason of any third person appearing to have claim or interest of the Subject
Property;

7. Finally, I hereby undertake to comply with all the rules and regulations of
MERALCO and agree to the disapproval of my application or disconnection of
electric services should I be found to be in violation of any of its rules and
regulations; and

8. I am executing this affidavit to attest to the truth of the foregoing statements,


and for whatever legal intents and purposes this may serve.
