I, Julie S. de Guzman, Filipino, of legal age, widow, and a resident of Bulanao, Tabuk
City, Kalinga, after having duly sworn to in accordance with law, hereby depose and
state:

1. That I am the mother of ____________ born in ______, on _______ as stated in


her birth certificate attached hereto as Annex “A”;
2. I am a widow for almost ________ and hence the only surviving parent of
_______;
3. That my daughter is intending to contract marriage with _________ born in
_____, on ________;
4. That I am giving my consent to my daughter, ________, marrying _______ and
that I have no objections whatsoever;
5. That I am executing this Affidavit for the purpose of informing the authorities
concerned of the veracity of the foregoing facts and for whatever legal purposes it
may serve.
