JOINT AFFIDAVIT OF COHABITATION

We, RINGGO LIM RODRIGUEZ, of legal age, Filipino, single, a resident of Lipa
City, Batangas and LUISA SOTOMAYOR, likewise of legal age, Filipino, single and a
resident of Lipa City, Batangas, after having been duly sworn to in accordance with
law, depose and say:

1. That we have been living together as husband and wife for six (6) years
already without the benefit of a lawful marriage;

2. That out of cohabitation, we begot one (1) child;

3. That long of cohabitation, we felt and desired by our own accord and
freewill to bind ourselves together into a legal marriage;

4. That there is no legal impediment of whatsoever from both of us to contract


marriage;

5. That we wanted to avail Article 34 of the Family Code of the Philippines and
avail the service of a Parish Priest, Presiding Judge or any solemnizing officer
to solemnize our marriage;

6. That we are executing this affidavit in order to prove the foregoing


statements and to support with our intention to get married legally under
Article 34 of the Family Code of the Philippines.
