AFFIDAVIT OF DESISTANCE

I, LORETO JACOB, of legal age, Filipino, married and resident of Brgy.


Belmonte, San Andres, Catanduanes, after having been sworn to in accordance
with law do hereby depose and state:

1. That I am the victim of a vehicular incident that happened on October


11, 2017 around 6:30 pm, certain ROMEL S. SOPRANES of Brgy.
Timbaan, San Andres, Catanduanes hit me by his motorcycle which
causes me physical injuries;

2. That Mr. Sopranes helped me in my medical expenses, thus the


issues between us are now settled;

3. That with the foregoing premises, I am no longer interested in filing a


case against him;

4. That I am executing this affidavit in order to attest to the veracity of the


foregoing statements and to affirm that the same are true and correct
to the best of my knowledge and belief, and for all legal intents and
purposes this may serve;

IN WITNESS WHEREOF, I have hereunto set my hand this 24 th day of


November, 2017 in Virac, Catanduanes.
