SUBJECT : Spot Report, re- Shooting Incident

DATE : May 15, 2022

1. On or about 7:30 PM of May 15, 2022, a shooting incident occurred at No. 93 Pluto St. Brgy.
Sta. Lucia, City of San Fernando, Pampanga, wherein victim, Kate Balingit, 24 y/o, single, a
resident of the mentioned address, was shot while buying on a sari- sari store, by the suspect,
Chris Crisostomo.

2. Hence, the victim sustained multiple gunshot wound in the different parts of the body and was
immediately brought to Jose B Lingad Hospital for medical treatment but was pronounced dead
on arrival by the attending physician thereat.

3. Cartridge cases were found at the crime scene and the suspect was not arrested due to the
fact that he has escaped the location where the shooting incident has transpired.

4. Apparently, the motive of the incident was jealousy. It was found out that the suspect was his
ex- boyfriend.

5. Cartridge cases have been collected by the SOCO personnel. Request for ballistic examination
to be sent to the PNP Crime Laboratory, and warrant of arrest for immediate apprehension of the
suspect. The case is still under investigation.

6. Progress report will follow.
