Task Description
Work Description Hazards Identified Preventive Measures
Obstruction  Secure approved permit and initiate Tool box talk prior to work.
Preparation of work area Slip/trip  Housekeeping before, during and after work completion
Elevation check; mark portion of Pinch point/crush  Designated surveyor do elevation checking
Foundation to be chipped Dust/ foreign body  Use proper PPE's & face shield during chipping
Prepare chipping tools
On eyes  Use of respiratory protection ( dust mask or half face piece respirator)
Commence chipping
 Chipping hammer grinding machine & other Power tools must be inspected
Prepare pads & grouting materials
(color coded)
Commence padding & grouting
QC inspection as coordinated  Provide fully charged fire extinguisher.
Rectification as required  Ensures safe access & platforms
Final QC inspection  Proper barricade and signage of work area
 Work is properly supervised & with safety coverage.
