AFFIDAVIT OF AUTHENTICITY

I, MARJORIE ACOYMO, of legal age, Filipino, married and a


resident Purok 13-B Papaya, Brgy. San Vicente, Butuan City, Agusan del
Norte, Philippines, after having been duly sworn to in accordance with law
hereby depose and state:

1. That incompliance for Certificate of Authority for a Notarial Act


(CANA) and Department of Foreign Affairs (DFA), I have
submitted my Experience Certificate as a Staff Nurse, issued by
the Aster Sanad Hospital;

2. That I am attesting the genuineness and authenticity of the


above-mentioned Experience Certificate;

3. That I hold myself liable for perjury, falsehood, misrepresentation


or omission or any act of dishonesty for any fraudulent, false,
and tempered document that I have submitted;

4. That I am executing this Affidavit to attest to the truthfulness of


the foregoing statements and for any other legal purpose/s it
may serve.
