AFFIDAVIT OF LOSS

I, CORAZON B. CRISTOBAL likewise of legal age, Filipino Citizen with


residence and postal address Blk 44 Lot 12 Garden Grove Vill. Salitran IV
Dasmariñas City Cavite, after having been duly sworn to in accordance with law,
depose and state THAT:

1. Sometime on September, 2012 the ETERNAL GARDEN MEMORIAL PARK


original CERTIFICATE OF OWNERSHIP with lawn : Eternal Faith-R Sec.
D Blk 9 Lots #5 & 6 was misplaced and got lost;

2. Recently, I had tried to look for the said CERTIFICATE OF OWNERSHIP but
the same could not be found;

3. Despite diligent search and efforts to locate the said CERTIFICATE OF


OWNERSHIP I could not find the same such that I believe that it is now lost
beyond recovery;

4. As such, I am executing this affidavit to attest to the foregoing facts and to support
the application for the issuance of a new one in lieu of the one which was lost.

March, 2019 in City of Caloocan, Philippines, Metro Manila, Philippines.
