AFFIDAVIT OF EMPLOYMENT

I, KRISTINE JOY C. LUENGO, of legal age, single, Filipino citizen, with


residence and postal address at No. 176 Golden Shower St., Payatas B.,
Quezon City, after having been duly sworn to in accordance with law do
hereby depose and say;

1. That I am a registered Medical Laboratory Technician of the Republic of


the Philippines;

2. That I worked as a Laboratory Technician from October 20, 2014


to March 1, 2018 at Bernardino General Hospital I. A copy of my
Certification dated March 2, 2018 is attached hereto and made an
integral part hereof;

3. That I worked as a Medical Technologist from May 1, 2018 to


March 8, 2019 at Val-Sia Family Clinic. A copy of my Certification
dated March 9, 2019 is attached hereto and made an integral part
hereof;

4. That I hereby attest to the authenticity and genuineness of the


document and I hereby agree to be liable and answerable to perjury
and/or falsification of public document in the event that the document
is found not in order;

5. That I am executing this affidavit to attest to the truth and veracity of


the above foregoing facts and for all other legal and purposes it may
serve.

IN WITNESS WHEREOF, I hereunto set my hand and sign on this 12 th


day of March 2019, at Quezon City, Metro Manila.
