AFFIDAVIT of denial with annexes


I, ___, Filipino, married, of legal age with office address at the _-_after having been
sworn to in accordance with law, do hereby depose and state that:
1. I am the presently
2. I requested from the Human Resources Department
of the above-mentioned corporation a Certification stating my present annual
compensation, hereto attached as Annex A;

4. I am fully aware of the corporations policy that an employee is entitled to
retirement/separation pay 3 months per year of service if he has served for 20 years, and
that employees who has served the corporation below 20 years are not entitled to
retirement/separation pay;
5. The said certification (Annex A) was issued to me for Visa purposes only
because I am presently applying for a permanent resident visa at the Canadian Embassy;
6. I have been with the corporation since 23 May 1994 and I am aware that if I
resign before accumulating 20 years of service, I will not be receiving any separation pay.
7. I am executing this affidavit to attest to the truth of the foregoing and for whatever
legal purpose this may serve.
