AFFIDAVIT

I, ________________________, Filipino, (indicate if single or married), of legal age,


and with office address at (insert address od DC Prop), after being sworn in
accordance with the law, do hereby depose and state that:

1. I am the Chairman of DC Properties Management Corp., a corporation duly


organized and existing under the Philippine Laws with principal office address
at __________________________;

2. That DC Properties Management Corp. (Employer ID no. 8003438749)


submitted a Social Security System Employment Report (R-1-A) dated 15
February 2024 which listed the names, specifically stated hereunder, and
declared below names as its employees:

3. The above mentioned employment with DC Properties Management Corp. did


not take effect nor did it push through as the stores where they were supposed
to be assigned to work were not able to open and start operations; and

4. I am executing this affidavit to attest to the truth and fact of the foregoing
statements in order that DC Properties Management Corp. may correct and/or
accomplish its submissions, requirements and compliances with the Social
Security System and for whatever other legal purpose this Affidavit may serve.
