AFFIDAVIT OF FAMILY COMPOSITION

I, CESAR R. CAMAÑA, of legal age, Filipino, married and residing at 725 San
Francisco St., Ilaya, Las Piñas City, after having been duly sworn to in accordance with
law, hereby depose and say:

1. That I am the legitimate child of Filipino parents, COSME F. CAMAÑA and


LEONILA S. RAMOS-CAMAÑA, born on June 18, 1969 in Las Piñas City;

2. That on May 8, 1993 I got married with TERESITA L. DELA CRUZ in


Parish of St. Joseph solemnized by Fr. Leo Renier, CIOM;

3. That by reason of said marriage, we begot child, whom we named and


christened as PATRICK COSME DELA CRUZ CAMAÑA, born on May 3,
1998 in Caytitinga III Alfonso, Cavite.

4. That I am executing this Affidavit to attest to the truthfulness of all the


foregoing facts and for whatever legal purposes this Affidavit may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 11 th day of


October, 2005 in the City of Las Piñas, Metro-Manila.
