feat: 新增 OAuth token 自動 refresh 機制與 usage API headers 修正

- Token 過期時自動 refresh（POST platform.claude.com/v1/oauth/token）
- Usage API 收到 401 時自動 refresh + 重試
- 跨平台 credential 儲存：macOS Keychain / Linux & Windows .credentials.json
- Token 讀取優先級調整為 env > Keychain > file（配合 v2.x 預設行為）
- 補齊 Usage API 缺少的 Content-Type 和 User-Agent headers
- 新增 6 個測試覆蓋 refresh、過期檢查、寫回等功能

feat: 新增 session 查詢功能與報表費用明細拆分

- session 命令新增 --session-id / --session-name 查詢（精確匹配）
- 解析 JSONL custom-title / agent-name 取得 session name
- 過濾模式顯示 per-source-file 明細（Session 大區塊 + Source File 逐行）
- Daily/Monthly 報表新增 Sessions 數量欄
- 所有報表新增 CC Cost / CR Cost / API Cost 獨立欄位
- Blocks 報表從單一 Tokens+Cost 拆分為完整 token 和費用明細

perf: add project-level incremental cache for blocks --live

Reduce CPU usage by 68% (26.7% → 8.5%) by caching per-project file state (ModTime+Size) and only reloading changed files. Includes 9 unit tests and documentation updates for v0.11.1.
