The handler asked for [FromRoute] profileName on a route without that parameter;
the generated request delegate throws InvalidOperationException, so every call
answered 500. Nothing in the suite called the endpoint. Handle() now takes no
profile and defers to HandleProfile(), mapped at /ca/{profileName}/csr like
/ca/{profileName}/crl. Two scenarios post a PEM request over both routes.
