docs: refine README header with a centered logo lockup - #9

Replace the plain # Kagan h1 (which GitHub underlines) with a centered logo-over-title lockup: the Kagan mark inside an <h3> with a <br> to the name, a one-line tagline, and the Docs · Quickstart links. Drop both horizontal rules from the header and the trailing rule at the bottom. The logo uses an absolute raw.githubusercontent URL so it also renders on the npm package page, where relative paths break. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>
