Track JSONL-parsed sessions separately from session discovery

Hook-driven session discovery only yields tool-call items, not the full
conversation text. Before this, once any hook had fired for a session,
isLoaded returned true and the JSONL parse would be skipped on first
chat open — leaving the chat view with partial content that wouldn't
update.
