@r12a pointed out that perhaps some guidance was missing that was in the TR version from 2011. These edits attempt to put in the ones that are not addressed by other refactoring of the doc. Notably, this includes the references to XPathFO (I corrected them to actually point to the right part of that spec too!!!) More to follow.
