AFFIDAVIT OF PARENTAL SUPPORT AND CONSENT

I, STEVE LAURENCE LEQUIDO, 38 years of age, Filipino citizen, and a


resident of Seaview Heights Subdivision, Lawaan I, Talisay City, Cebu, after being
ssworn to in accordance with law, do hereby depose and say:

That I am the biological parent of SETH ELEANDRE LEQUIDO, 12 years old,


Filipino citizen, and is presently residing at Seaview Heights Subdivision, Lawaan I,
Talisay City, Cebu;

That I am working as a freelance Service Engineer and currently Self-


employed;

That I cannot personally travel with my child and accompany him to New
Zealand, moreover I hereby give full consent to the travel of the aforementioned
child alone or to b accompanied by RIECHAN VILLALUZ;

That I’ve already secured the necessary permits, travel documents,


Passport, DSWD Clearance and visa, etc.;

That I further guarantee that the child will not be in any manner become
public ward of any authority of the Philippines or any country the child/children
may happen to pass by enroute to NEW ZEALAND;

That I will be supporting my child’s incidental fees, airfares and


accommodation during my child’s stay in New Zealand;

That I am executing this affidavit to verily affirm my consent and


agreement. I further attest to the truthfulness of the foregoing facts.

IN WITNESS WHEREOF, I/we have hereunto set my/our hand this 29th day of
August, 2018 at Cebu City, Philippines.
