LETTER OF ACCEPTANCE

September 22, 2020

Sir/Maam:

Greetings!

This is connection with one of our residents who is locally stranded and is
subject for repatriation.

In view of this, I HEREBY allow JUNRHEY B. DUMAS a resident of Brgy. Ipil,


Gigaquit, Surigao del Norte to enter in our municipality.

This Travel Acceptance is subject to the following condition:

a. Said person/s shall undergo a Rapid Antibody Test upon arrival in any of the provincial
checkpoints of this Province; and

b. If tested NEGATIVE in Rapid Antibody Test, they/he/she will then proceed to the Barangay/
Municipal Isolation Unit/s to undergo 14-day quarantine. On the other hand, if tested POSITIVE
in Rapid Antibody Test, they/he/she will undergo swan test RT-PCR Test, and while waiting for
the result, they/he/she shall stay at the Municipal Isolation Unit/s.
