AFFIDAVIT OF LOSS

I, CHRISTOPHER ESCOTOTO LEQUIN of legal age Filipino, with residence address at Blk
27 Haggai St., Brgy. San Juan, Exodus Floodway, Cainta, Rizal, after having sworn to in
accordance with law do hereby deposes and state:

1. That I am the owner/possessor of High School Diploma (S.Y. 2000), Form 137, and CAT
Certificate issued by Camp Gen. Emilio Aguinaldo High School in Cubao, Quezon City;

2. That sometime in 2005 the above mentioned items were inadvertently lost or might
have been misplaced due to unknown circumstances;

3. That despite diligent efforts to locate the same all efforts proved fruitless up to the
present and fear that the same were lost beyond recovery.

4. That I am executing this affidavit to attest to the truth of the foregoing instrument and
for any legal purpose and intents this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 21 st day of June, 2021 in
