Dear Engr. Dela Cerna,

Good day!

The door jamb in our cashiering division has been eaten by termites which cause
damage not only to the wood but also to the wirings of the Air Conditioner adjacent to the
door jamb. We seek out help from the CGSO about this but they only help us in fixing the air
conditioner, they advised us to ask for your assistance with regards to the door jamb. Thus,
we are writing this formal request to humbly ask for help from your good office. Herewith, is
a picture of the damage door jamb.

Furthermore, we also humbly ask you to send us some of your skilled personnel to
help us transfer the frames of our Citizens Charter. At present, our Citizens Charter are
placed on the top of our window counters which are barely seen by our clients, we want to
transfer these to the wall in front of our office for better view.

We earnestly hope that you will grant our humble request.
