AFFIDAVIT

I, HAMIDA RASUL KAMID, Filipino, of legal age, and a


resident of Sitio Tapayan, Brgy. Sta. Ana, Taytay Rizal,
having duly sworn to in accordance with the law, do hereby
depose and state:

1. That I am an Overseas Filipino Worker working as a


Domestic Helper;
2. That I have applied to be an Overseas Filipino
Worker to work as a domestic helper at the overseas
employment agency named Century High which has
their office at Mabini, Metro Manila;
3. That I was issued with a 21-VISA working visa for
domestic helpers reflecting to that indicated in my
employment contract;
4. That I arrived in Doha, Qatar on August 7, 2015;
5. That I worked in Qatar as a domestic helper;
6. That my sponsor in working in Qatar was my
employer, Mr. Tayseer Husni-Abunada;
7. That I worked with Mr. Husni-Abunada for Two(2)
years and two(2) months;
8. That Mr. Husni-Abunada is still going to be my
employer upon my return to Qatar;
9. I met my employer when Mr. Husni-Abunada went to
the employment agency to fetch me;
10. That my first employer before Mr. Husni-Abunada
opted to send me back to my employment agency,
and so the agency had to find another employer to
take me in as a domestic helper;
11. That I am currently employed as a domestic
helper with Mr. Tayseer Husni-Abunada as my
employer;
12. That the monthly salary I receive from Mr. Husni-
Abunada is 1,500 Riyals;
13. That I have read, understood and accept the terms
and conditions embodied in my employment
contract;
14. That, I have executed this affidavit to declare all
stated herein as well as attest to the truth of the
foregoing facts;

IN WITNESS WHEREOF, I have hereunto affixed my


signature on this 30th day of April 2018 in Pasig City,
Philippines.
