AFFIDAVIT OF SUPPORT
for TRAVEL EXPENSES
I, (NAME NG ASAWA NI MAAM JOVEY), of legal age, Filipino citizen, married and
residing at Centro East, Sta. Teresita, Cagayan, after being first sworn according to law, hereby
depose and say, that:
1. I am the Lawful husband of Jovey De Gracia-Eduria, a holder of passport bearing

2. She will be travelling to South Korea ___________________ up to
_______________________;
3. I will be responsible for my wife’s accommodation, subsistence, and all other
expenses for the said travel;
4. I guarantee tht my wife will not violate any laws of the said country, or work or seek
employment thereof;
5. My wife will return to the Philippines on or before the expiration of the period of her
authorized stay in South Korea. I understand that overstaying, working without a
valid permit and pimping are crimes in the said country;
6. I execute this Affidavit to attest the veracity of the foregoing and in support for the
VISA application of my wife and for whatever lawful purpose it may serve.
