AFFIDAVIT OF PARENTAL CONSENT TO MARRIAGE

I, MARIA C. TAPIZ, of legal age, married, Filipino, and a resident of Brgy. San
Roque, Calbiga, City, after having been duly sworn to in accordance with law do hereby
depose and state: That -

1. I am the mother of Joanne C. Tapiz, 38 years old, born on 23


January 1985 in Calbiga, Samar;

2. My daughter intends to contract marriage with Mr. Toben Verrghese,


an Indian citizen, a resident of A6-10D, Olympia Grande Apts, Gst Road,
Pallavaram, Chennai, India;

3. I already personally talked with my daughter about her plan to get


married;

4. Thus, I am giving my consent to my daughter marrying Mr. Toben


Verrghese especially since she is single and already at the right age;

5. I am executing this statement to attest the truth of all the foregoing


and for legal purposes.
